Norwescap is partnering with the Warren County Health Department to provide support to COVID-positive and COVID-exposed persons identified in the county. This position will play a critical role in ensuring that individuals affected by the pandemic in Warren County can connect into relevant resources including housing, health, and financial well-being. Specific duties include:
o Develop and maintain a list of available resources to assist COVID-positive and COVID-exposed persons, with emphasis on services available within the county.
o Identify vulnerable populations in the LINCS jurisdiction and, working with the LINCS
o COVID Coordinator/LHDs and in alignment with State testing and containment
strategies, prioritize COVID response initiatives to target the vulnerable populations in
o Develop and maintain a list of pre-identified isolation and quarantine sites available
within the county and develop a standard operating procedure (SOP) for LINCS/LHDs
and local partners to use for persons in need of isolation or quarantine services.
o Liaison with the DOH Quarantine Director to communicate the State SOP for Medical
Quarantine Sites (MQS) to local partners and, as needed, develop local policies and
procedures that support the State SOP.
o Work collaboratively with LINCS/LHDs and contact tracers to support needs identified in
their communication with COVID-positive and COVID-exposed persons.
o Identify gaps in needs amongst COVID-positive and COVID-exposed persons,
especially those in vulnerable populations (e.g., persons experiencing homelessness,
immigrants, persons of color) and search for local solutions. This could include, but is
not limited to, food banks, mental health services, visiting nurses, telehealth solutions,
and other community programs that can address the needs of COVID-positive and
o Work with the DOH Regional SSC to establish and maintain contact with LINCS SSCs in
other counties to ensure local, regional and statewide coverage and awareness of
o Participate in any COVID task force, working group or other meeting held by the LINCS
agency, LHDs, Regional Coordinators, DOH Quarantine Coordinator and DOH SMEs
that may be relevant to social supports.
o Complete NJDOH’s CommCare training and assist the Health Department as needed with COVID-19 case investigation and contact tracing.
o Other duties as assigned.
Rate of Pay: $20
Required Level of Education: Bachelor’s degree in social work, sociology, psychology or other social science.
