Job Title: Community Health Worker
Summary:
The Community Health Worker will provide basic lay navigation to age-eligible insured, underinsured, and uninsured individuals and without medical homes to eligible primary care, cancer screening and, COVID-19 and Influenza vaccination sites. The Community Health Worker supports the Health Connections mission by identifying and helping to reduce barriers to healthcare and vaccinations. Gathers and produces community health needs assessment data on community barriers to health. Provides referrals to necessary resources (i.e. Healthcare professionals), educational materials for cancer risks, COVID-19 and Influenza immunization, and transportation support.

• Addresses gaps in care of clients – effectively work with individuals from diverse backgrounds in reducing cultural and socioeconomic barriers between clients and institution
• Assesses and identifies actual and potential clients problems and barriers to care by gathering relative data; Identifies and assists clients in reducing barriers to care
• Conducts outreach activities on COVID-19 and Influenza Immunization
• Educates and assists clients in the management, prevention, and maintenance of their health and wellness
• Builds and maintains relationships with both internal and external healthcare contacts and organizations in an effort to improve available client resources
• Collaborates with members of the Health Connections team to identify concerns that would prevent quality medical care; Communicates client plans and concerns to healthcare providers
• Establishes positive, culturally aware, supportive relationships with clients in managing their health care needs
• Tracks client outcomes and provides information to authorized personnel as requested, using designated electronic devices and software tools
• Provides referrals for services to community agencies and internal case management as appropriate
• Connects vulnerable community members to Engagement Partners and Patient Navigators for continued care when needed; assists with case management
• Acts as a client advocate and liaison between the community service agencies (i.e. schools, Department of Health, hospitals, support groups, etc.)
• Conducts outreach, contacts churches and other civic, community groups, distributes posters, flyers and brochures and conducts one-on-one education at every opportunity
• Hosts community outreach events surrounding vaccine and health equity under supervision of Health Connections Coordinators
• Ensures confidentiality of client information
• Fields phone calls in Health Connections office when present and connect patients to resources
• Support office management tasks
• Assist with patient data entry
• Assist Program Manager with program reports and presentations

Preferred Qualifications:
• Must be a NJ resident
• Minimum HS Diploma or GED
• Bachelor’s Degree preferred
• Minimum one year of community involvement and/or working with clients, or an equivalent combination of education and experience
• Knowledge of community resources
• Demonstrates proficient use of computers, including Microsoft Suite as well as perform web searches for clients; excellent verbal and written communication skills with focus on quality customer service
• Bilingual English/Spanish required
• Ability to work both independently and as part of a team; Strong organizational and interpersonal skills
Documentation:
• Valid NJ driver’s license and reliable transportation
• Provides written evidence of vehicle insurance of any vehicle the incumbent drives for job-related purposes
• Maintains vehicle insurance of such vehicle(s) while employed
Working Environment/Demands:
• List works indoors – 40%
• List works outdoors – 60%
• Must be able to lift at least 15lbs
